John Zastawrny

John Zastawrny, PT, DPT

Chiropractor/Founder of Integrated Chiropractic

With over 25 years of clinical experience, John Zastawrny, PT, DPT, founded Integrated Physical Therapy with one clear mission: deliver results-driven care that helps patients move better, feel stronger, and return to the activities they love—without unnecessary delays.

A graduate of Damen University (2000), John earned his Doctorate of Physical Therapy in 2008 and has built his practice around highly skilled, hands-on manual therapy combined with individualized treatment planning. His approach focuses on identifying the true root cause of pain—not just treating symptoms—so patients experience lasting improvement.

John specializes in treating a wide range of orthopedic conditions, including:

  • Athletic and sports injuries
  • Post-operative rehabilitation
  • Neck and back pain
  • Shoulder, ankle, elbow, wrist, and hand injuries
  • TMJ (jaw) dysfunction

By combining advanced manual therapy techniques with targeted corrective exercise, John helps patients reduce pain faster, restore mobility, and confidently return to work, sports, and everyday life.

He has pursued extensive post-graduate training in advanced treatment methods, including the Mulligan Concept, Graston Technique, McKenzie Method, Primal Reflex Release Technique (PRRT), coursework with Stanley Paris, Associated Awareness Technique, and numerous other specialized programs. His commitment to continuing education ensures that patients receive modern, evidence-informed care tailored to their specific goals.

Outside the clinic, John is a dedicated husband to Kellie and father to Grayson and Kathryn. As an active golfer, runner, and fitness enthusiast, he understands firsthand the importance of staying active—and brings that same passion to helping his patients do the same.

Jacob Brackett, DPT

Physical Therapist

Jacob graduated from the University at Buffalo in 2017 with a degree in Exercise Science before earning his Doctor of Physical Therapy from Concordia University, St. Paul in Minnesota in 2022. During his time in PT school, he served as class president and founded the research club within the PT department. He graduated with honors and went on to pursue advanced orthopedic training through a residency program, earning his Orthopedic Clinical Specialist (OCS) certification.

Jacob specializes in orthopedic physical therapy, combining hands-on manual therapy with individualized exercise prescription to treat a wide range of conditions, from acute injuries to chronic pain. He has completed extensive continuing education in areas including vestibular rehabilitation, vertigo, balance disorders, TMJ-related pain, and sciatica. He is also able to provide care to Spanish-speaking patients, helping make quality rehabilitation more accessible to a broader community.

In addition to his orthopedic background, Jacob has experience as an adaptive physical education teacher, where he worked with children with developmental disabilities, primarily autism. He continues to draw on this experience in his physical therapy practice, helping individuals with developmental disabilities improve their quality of life, build independence, and develop confidence with functional activities.

At the center of Jacob’s approach is a strong emphasis on wellness and long-term health. Rather than focusing solely on treating an injury or managing symptoms, he strives to help each patient build the strength, mobility, and confidence needed to live a longer, healthier, and more fulfilling life.

Outside of the clinic, Jacob enjoys playing disc golf and basketball, playing guitar, and spending time with his growing family. He is passionate about staying connected to his community and looks for opportunities to give back to the people and communities that have helped shape his own journey.
